The MHIP application is used for both the state and federal pools. Please thoroughly complete all applicable areas.

After you have completed and signed the application, consult the application checklist to make sure you have included all required documentation and are mailing all documents to the correct address. Incomplete documentation will delay the processing of your application.
Once your application and documentation are reviewed, you will receive written notification that you have been accepted into the state or federal pool.
